Class of 2019 Students must have either:
- 2 doses of meningococcal vaccine - the 2nd dose must have been given on or after age 16
- 1 dose if your child’s first dose was given on or after age 16
Please submit proof of the required vaccine by June 1, 2018 so there will be no delay in your child starting school on time in September. This proof must include:
- Name of the vaccine
- Date vaccine given
- Who gave it, along with their title; or where it was given if at a clinic
Students will not be permitted to start classes in September 2018 until this proof is submitted. If you have questions, please contact Nurse Kathy DeMello during regular school hours. After June 22, 2018, the Nurse’s summer hours are Thursdays only from 9 AM – 12 noon.
