A new sports physical and medical clearance card is required before try-outs; the medical examination must be within the past year. The sports medical form can be downloaded from the Athletics home page. Please submit all medical forms to the main office no later than Thursday August 31st. No one is permitted to try-out for a sport without first obtaining a Medical Clearance Card from the Nurse.
