The following terms and conditions form part of this Transportation contract.

The Parent/Guardian acknowledges and agrees that the student must comply with the School Transportation Safety Rules and School Transportation Protocol while being transported to school.
The Parent/Guardian/Student has read and agrees to abide by the St. Edmund Prep Code of Conduct as outlined in the student handbook. The SEP Code of Conduct applies to all students while waiting for and while riding any form of SEP transportation.
The Parent/Guardian is responsible for the safety of their child travelling to the designated pick-up stop.
The Parent/Guardian understands that no changes shall be made regarding specific pick-up locations and times without prior written request.
The Parent/Guardian agrees that transportation will not be guaranteed if the completed contract is not returned on or by August 31, 2024.
The Parent/Guardian promises to pay the annual fee referred to in this Contract. There are two payment options.
1. Single Payment: A single payment of $1,025 due by August 31, 2024
2. Monthly Payments: Ten monthly payments ($110 per month) beginning in September and added to the student’s FACTS account.
If the Parent/Guardian permanently moves his/her residence after the beginning of the school year, and as a result the student no longer requires transportation services, or the student moves to another school and no longer requires transportation services, a prorated refund, less an administration fee of $40 shall be made upon the receipt of the Parent/Guardian's request in writing. Requests must be made within 30 days of the last day of busing and prior to April 30, 2025. No refunds will be processed for cancellation requests received after April 30, 2025.
The Parent/Guardian understands this is a ten-month commitment. Should a student decide to stop riding the bus, the Parent/Guardian is still responsible for the monthly payment.
Transportation assignments shall not be transferable or assignable to any third party.

Student Transportation Agreement

Students must:

Be ready at their transportation stop five minutes before the vehicle arrives.
Remain on the vehicle until their destination has been reached.
Be ready for and promptly get off the vehicle when their destination has been reached.
Remain seated while the vehicle is in motion and sit facing forward with legs, arms and hands in the seat and not in the aisles or out windows.
No food or drink in the vehicle.
Address the driver with courtesy and respect and follow their direction; do not distract the driver.
Show courtesy and respect to all riders; refrain from the use of inappropriate language.
Refrain from yelling or speaking loudly.
Refrain from throwing objects in the vehicle or out the vehicle windows; refrain from harassment and bullying of any kind.
Treat the vehicle with respect and report any damage.
Honor and follow the student policies as outlined in the Student Handbook.

Vehicle protocols:

The vehicle will leave its designated pick-up location three minutes after arrival.
Students who are late will be left behind.

STUDENT AGREEMENT:

I understand that riding in the SEP school vehicle is a privilege and not a right. I understand that I am responsible for my conduct while in the vehicle. I acknowledge that the school vehicle driver is responsible for my safety and transportation, and I am accountable to the vehicle driver while riding the vehicle. I understand that violating this agreement will result in disciplinary action and possibly a loss of vehicle riding privileges. Choosing "I agree" indicates that I have read the following Student Transportation Agreement, and I am committed to honoring and above stated the rules.
